Enrollment Application Form

I hereby authorize California Credit Union to issue a pay card that will be used to load my payroll. I further authorize the Los Angeles Unified School District or its agents to fund (and/or correct previous deposits) to the pay card through the account that will be created for my pay card. California Credit Union is authorized to deposit and/or correct the amounts to my pay card which is linked to this account. I must submit a new authorization form if I change or close my pay card. This authorization replaces any previously made by me and is to remain in effect until a new authorization card is submitted by me.
